A procedure on the border between reconstructive and aesthetic surgery
Abdominoplasty — or tummy tuck — removes excess skin and fat from the abdomen and tightens the muscular wall. It is most often sought by patients after one or more pregnancies, or after significant weight loss.
Depending on the situation, the same procedure may qualify as reconstructive surgery (partially covered) or as aesthetic surgery (entirely at your expense). Everything hinges on precise medical criteria.
The main criterion: the abdominal apron
The French public health insurance (Sécurité sociale) may cover an abdominoplasty when the excess skin forms a true abdominal apron hanging over the pubis. This is the classic qualifying criterion, because such an apron has functional consequences: maceration and skin irritation in the fold, recurrent fungal infections, difficulty getting dressed and exercising.
Other elements strengthen the application:
- Documented massive weight loss (particularly after bariatric surgery)
- Recurrent, medically documented skin lesions in the fold
- A weight that has been stable for at least six months
Conversely, simple skin laxity, localised excess fat or moderate muscle separation (diastasis) without an apron generally do not meet the coverage criteria.
The practical steps
- Consultation with a plastic surgeon, who assesses whether your situation meets the criteria and prepares the application file (medical photographs, clinical description).
- Prior approval request sent to the medical officer of the French health insurance fund when the situation requires it; you may be called in for an examination.
- The fund's response: under the French prior-approval procedure, no reply within the statutory period counts as approval.
Your surgeon will tell you from the very first consultation whether an application has a reasonable chance of success — there is no point in building a file that does not meet the criteria.
Coverage does not mean it is free
Even when approved, coverage is based on the standard rates of the French health insurance system:
- The surgeon's and anaesthetist's fees may include surcharges, reimbursed or not depending on your supplementary (top-up) health insurance
- Hospital comfort charges remain at your expense
- In practice, out-of-pocket costs ranging from a few hundred to a few thousand euros are common, depending on your supplementary insurance contract
Always ask for a written quote detailing the reimbursable portion and the surcharges, and send it to your supplementary insurer before making a decision.
What if the procedure is aesthetic?
Without qualifying criteria, abdominoplasty is an aesthetic procedure: in Paris, expect generally between €6,000 and €10,000 all-inclusive depending on the extent of the surgery (full abdominoplasty with repositioning of the navel, mini-abdominoplasty, combined liposuction).
The 15-day cooling-off period required by French law applies, as for any aesthetic surgery.
Preparing for surgery under the right conditions
Whichever path applies — reconstructive or aesthetic — the prerequisites are the same:
- Stable weight for several months: abdominoplasty is not a weight-loss method
- Stopping smoking before and after the procedure, essential for proper healing
- Pregnancy plans: it is better to operate after the last planned pregnancy
