Liposuction or tummy tuck: how do you choose?

Liposuction treats excess fat; abdominoplasty treats excess fat and excess skin. The right choice depends on three simple criteria: your skin quality, the presence of diastasis and your future plans.

Two procedures, two different problems

Excess fat around the abdomen that does not reduce despite a balanced diet and regular physical exercise often leads to considering two procedures, in high demand and frequently confused:

  • Liposuction treats localised excess fat, suctioned out through very short incisions. It does not remove any skin.
  • Abdominoplasty (tummy tuck) treats excess fat and excess skin: it removes the surplus skin, tightens the muscle wall if necessary, and is most often combined with liposuction.

An essential point: these operations do not aim to correct excess weight, but the silhouette. They are performed when BMI is normal, or when the patient has reached the weight goal they set themselves. The right choice between the two depends not on your preferences, but on your anatomy.

Criterion 1: skin quality

This is the decisive criterion. After liposuction, the skin has to retract over the new contour, like an elastic fabric.

  • Firm skin, no major stretch marks: liposuction alone gives a good result
  • Stretched skin, numerous stretch marks, a visible skin fold when seated: liposuction alone risks worsening the 'empty skin' appearance — abdominoplasty is the answer

A simple test gives a first idea: pinch the skin below the navel. If it is thin, supple and springs back immediately, it will retract well. This test obviously does not replace a clinical examination.

An intermediate case: when the excess skin is not too significant and skin elasticity allows it, liposuction can be combined with radiofrequency treatment to stimulate skin retraction — thereby avoiding the scar of an abdominoplasty. Pr Garrido uses Quantum RF 25 technology for this.

Criterion 2: rectus muscle diastasis

After pregnancy in particular, the rectus abdominis muscles can remain separated (diastasis): the abdomen bulges even without excess fat, and no amount of core training corrects it completely when the gap is significant.

Liposuction has no effect on diastasis. Only abdominoplasty makes it possible to retighten the muscle wall during the same operation.

Criterion 3: your plans and constraints

  • Pregnancy planned: better to postpone an abdominoplasty until after your last pregnancy; moderate liposuction can sometimes be considered, to be discussed
  • Recovery: abdominal liposuction, about 1 week off work and a compression garment for 4 to 6 weeks; abdominoplasty, 2 to 3 weeks off work and a scar above the pubic area to accept
