Breast reduction: eligibility and reimbursement in France

Breast reduction is one of the few breast procedures covered by the French public health insurance, under certain conditions — notably the removal of at least 300 g per breast. Here is how it works in practice.

A functional operation before an aesthetic one

Breast hypertrophy is not just a matter of silhouette: back, neck and shoulder pain, irritation grooves under bra straps, moisture and chafing in the inframammary fold, discomfort during sport, difficulty finding clothes that fit. It is because of these functional consequences that breast reduction can be covered by the French public health insurance (Assurance Maladie).

The main condition: around 300 g per breast

The usual criterion for coverage is the removal of at least 300 grams of glandular tissue per breast. It is the surgeon who, on examination, estimates whether your hypertrophy reaches this threshold — and tells you clearly from the very first consultation.

Below this threshold, the operation is considered an aesthetic request (a breast lift with moderate reduction, for example): it is then not reimbursed and costs from €6,500 in Paris — see the full fee schedule.

Coverage does not mean zero out-of-pocket cost

When the criterion is met, the operation is covered on the basis of the standard national rates, without prior authorisation in most cases. You should still anticipate:

  • Extra fees charged by the surgeon and the anaesthetist above the standard rate, common in the French "sector 2" fee system — reimbursed partly or fully depending on your complementary private insurance (mutuelle)
  • Comfort costs (private room)
  • A written quote is always provided: send it to your complementary insurer before scheduling the operation

In practice, depending on your insurance contract, the out-of-pocket cost can range from a few hundred to a few thousand euros.

The questions every patient asks

What scars?

Most often an inverted-T pattern: around the areola, vertically down to the fold, and along the inframammary fold. In some moderate cases of hypertrophy, a vertical technique limits the scar in the fold. Scars are red for the first few months and then fade over 12 to 18 months — appropriate care (scar treatments) can support this maturation.

Will I be able to breastfeed?

Breastfeeding is not contraindicated after breast reduction: the technique preserves the attachment of the areola to part of the gland. It can however be disrupted by this surgery and force breastfeeding to stop. If a pregnancy is planned in the near future, it is often better to discuss the timing during the consultation.

Is there a minimum age?

We generally wait until breast growth is complete. In a young patient who is severely affected, the operation can be considered early, in consultation with the parents and the family doctor.

What is recovery like?

Day surgery or one night in hospital, moderate pain that is well controlled, 5 to 15 days off work for most jobs, a support bra for about 6 weeks, and a gradual return to sport from 4 to 6 weeks.

Reduction, ptosis, asymmetry: different cases

Coverage follows different rules depending on the request: correction of pure ptosis (sagging breasts without excess volume) is not reimbursed, while a major asymmetry may be.
